Pre-visit information

This page contains information we are required to provide to you before your visit. When you arrive at DEC for your appointment, we will ask you to sign acknowledgement of receipt of our policies. Before your visit to DEC, please take time to review the policies on this page.

Policies

Commercial Insurance Policy

As a courtesy to you, we will file your primary insurance; however, it is essential that we receive accurate and timely information. Our office will be happy to re-file insurance one (1) additional time if we do not receive a response within 45 days. After a 60 day period, it will become your responsibility to follow up with your carrier to determine why the claim has not been paid. Regarding secondary insurance coverage, our office will be happy to submit a claim for you.

Medicare/Medicaid Policy

Medicare

Our office accepts assignment on Medicare. What this means is that we have agreed to accept Medicare's allowed amount as full payment however, Medicare only pays 80% of the approved amount leaving a 20% co-payment on the approved charges. The 20% is the responsibility of the patient. Screening coverage with Medicare may have different coverage amounts.

In addition, patients will be responsible for the Medicare deductible if it has not been met.

Medicaid

We do accept Medicaid patients by physician referral only. Medicaid patients will be responsible for bringing a signed, valid card with them to all office visits.

Managed Care Policy

Dallas Endoscopy Center, LLC, participates in a number of managed care plans; however, please be aware that there may be different reimbursement schedules for in and out of network providers. If you are unsure if we participate in your particular plan, please be sure and check with us before scheduling an appointment.



Advanced Directives

To compley with the Self-Determination Act (PSDA) and state law and rules regarding Advance Directives, Dallas Endoscopy Center requires each patient to sign the Texas Advance Directives document prior to their scheduled procedure. Advance Directives are statements that indicate the type of medical treatment wanted or not wanted in the event an individual is unable to make those determinations and who is authorized to make those decisions. The Advanced Directives are made and witnessed prior to serious illness and injury.

There are many types of Advanced Directives but the two most common forms are:

Living Wills

These generally state the type of medical care an individual wants or does not want if they become unable to make their own decisions.

Durable Power of Attorney for Health Care

This is a signed, dated and witnessed paper naming another person as an individual's agent or proxy to make medical decisions for that individual if they should become unable to make his or her own decisions.

In the ambulatory care setting, if a patient should suffer a cardiac or respiratory arrest or other life-threatening situation, the signed consent implies consent for resuscitation and transfer to a higher level of care. Therefore, in accordance with federal and state law, the facility is notifying you it will not honor previously signed Advanced Directives for any patient. If you disagree, you must address this issue with your physician prior to signing this form.

Physician Ownership

The physicians who practice in this facility are Board approved and licensed in the State of Texas. You may request a review of credentials from our Administrator.
